We strive to meet the needs of our patients on an ongoing basis. Some of these policies have been put in place to keep costs down for our patients.
Our office delivers a courtesy reminder call for your upcoming appointments, but it's the responsibility of the patient to remember. Appointments must be canceled 24 hours prior to the scheduled time. A fee will be required for cancellations within a 24 hour period.
If you require a medication refill, contact our office to request a possilbe refill. All controlled medications require an appointment for a refill.
All information given to our office is confidential. Your medical information will only be discussed with you unless and until we have written permission from you. Your insurance company does, however, have the legal right to copies of your medical progress notes.
Patient responsibilities are due at the time of service. In an event of a high balance due to a hospital stay we do offer a financial payment plan. We accept Mastercard, Visa, Debit Cards and cash.
